Includes manual and Quick Start guide as shown. Drivers not included, but I can find them for you and email them for free or burn them to a CD-R for an additional $1. The slot cover/IO plate is gold plated. VERY cool! Unlike some other sellers, my definition of "tested" means more than putting it in a computer, turning it on, and seeing if it's recognized. My testing process is to install it in a Windows XP machine, install drivers for it, and test it in different resolutions, checking for corruption, incorrect colors, and screen banding. If there are any problems encountered in this process, it doesn't get listed.
